DAFI91-204_SPOCSUP revises common core investigation requirements and aligns guidance with new reporting requirements in the AF Safety Automated System. It also establishes terminology changes for safety reports and exhibits and identifies waiver authorities by Tier number.
Applies to: This publication applies to United States Space Force (USSF) Headquarters (HQ) Space Operations Command (SpOC) units.
The reporting requirements in this directive are exempt from report control symbol (RCS) licensing in accordance with AFI 33-324, The Air Force Information Collections and Reports Management Program.
Subordinate units will send a copy of supplements to HQ SpOC/SE (SPOC.SE.Workflow@spaceforce.mil) for review and coordination prior to HAF Safety routing and approval.
Failure to observe the prohibitions and mandatory provisions in paragraph 4.4 by Regular AF members, USSF members, AFR members in an active or inactive duty for training status, and ANG members in federal Title 10 status is a violation of Article 92, Uniform Code of Military Justice (UCMJ).
The authorities to waive requirements in this publication are identified with a Tier ("T-0, T-1, T-2, and T-3") number following the compliance statement.
