It describes naval message reporting, defect and malfunction reports, and the data needed for analysis and evaluation. It requires reporting by the organization experiencing the malfunction or defect and addresses actions during combat operations, investigations, and ammunition disposition.
Applies to: The instructions contained herein are applicable to reporting malfunctions and/or defects associated with Class V(W) materiel under the management cognizance of Marine Corps Systems Command, Program Manager for Ammunition, (MARCORSYSCOM (PM Ammo)).
All ammunition malfunctions and defects require reporting in accordance with enclosures (1) through (4) by the organization experiencing the malfunction or defect. (paragraph 4)
Commanders at all echelons shall ensure the provisions of this Order are understood by all personnel involved in handling/expending ammunition. (paragraph 4)
Weapon will be tagged "out-of-service" and maintained in "as-is" condition, pending possible investigation. (paragraph 4)
Unsafe ammunition will not be transported or returned to the ASP. (paragraph 5)
