What form and documents are required for a SAAM request?

The unit is required to accomplish the basic special assignment airlift mission (SAAM) request using the DD Form 1249, Airlift Request – SAAM or Joint Chiefs of Staff (JCS) Exercise format and an aircraft load plan. (paragraph 1)

Where must airlift requests be submitted, and what format is required?
