The request will contain, as a minimum, a memorandum from the requestor addressed to the requestor’s commander and 28 SFS/S5R, in turn. (paragraph 5)
Once approved or disapproved by the Installation Traffic Review Officer, 28 SFS/S5R will notify the requestor’s commander via telephone or e-mail within one duty day. (paragraph 5)
Increase by two years the suspension or revocation period when someone is discovered driving in violation of their original suspension or revocation. (paragraph 5)
No points are assessed for revocation or suspension actions. (paragraph 5)
Reports of moving traffic violations recorded on DD Form 1408 will serve as a basis for determining point assessment. (paragraph 5)
