What form is required for munitions-related requests, and when must requests be sent?

Utilize the EAFB Form 5088 (Figure 1) when making munitions-related requests. These requests should be sent to the Munitions Scheduler NO later than 10 duty days before the support is needed. Forward any requests for short-notice support to Munitions Flight Supervision for approval. (paragraph 1)

How soon must expenditure requests be submitted?

Submit Expenditure requests via the Edwards FV2805 Organizational e-mail account within 2 duty days of use. (paragraph 1)

What information must be included with CAD/PAD turn-in requests?

Submit Turn-In requests via the Edwards FV2805 Organizational e-mail account NO later than 10 duty days of requesting date to include item Date of Installation/Removal, Lot Number, Serial Number and type of Turn-In, i.e. Custody, Consumption, Due-In-From Maintenance, etc. for CAD/PAD items. (paragraph 1)

What are the requirements for custody account inventory count sheets?

Submit Custody Account Inventory count sheets generated through the Theater Integrated Combat Munitions System (TICMS) within 15 calendar days. Legibly enter quantities for all lines, two individuals must perform the physical count(s) and manually sign and return the count sheets via the Edwards FV2805 Organizational e-mail account. Digital entries and signatures will not be excepted on count sheets. (paragraph 1)

When are issue requests due, and when can issue pickups occur?

Submit Issue Requests via the Edwards FV2805 Organizational e-mail account NO later than 10 duty days of requesting date. Issue pickup dates are always Tuesday and Thursday of a given week. (paragraph 1)
