(AMC) This publication supplements DAFMAN 91-223, Aviation Safety Investigations and Reports, to provide Air Mobility Command (AMC) policy on aviation mishap investigation and reporting.
Ensure all records generated as a result of processes prescribed in this publication adhere to AFI 33-322, Records Management and Information Governance Program, and are disposed in accordance with the Air Force Records Disposition Schedule, which is located in the Air Force Records Information Management System.
The authorities to waive wing or unit level requirements in this publication are identified with a Tier (“T-0, T-1, T-2, T-3”) number following the compliance statement.
Major changes were made to align guidance with the investigation reporting requirements in the AF Safety Automated System (AFSAS) https://afsas.safety.af.mil.
Common core investigation requirements have been moved to DAFI 91-204.
