DAFMAN91-222 addresses space mishap classification, on-orbit anomaly and event reporting, hazard reporting, investigation membership, evidence collection, and safety report procedures. It also sets requirements for records, waivers, supplements, and information protected by the Privacy Act of 1974.
Major changes include adding United States Space Force language where appropriate, clarifying research, development, test, and evaluation reporting requirements, clarifying on-orbit anomaly reporting requirements, and adding investigation and safety report procedures.
Ensure all records generated as a result of processes prescribed in this publication adhere to Air Force Instruction (AFI) 33-322, Records Management and Information Governance Program, and are disposed in accordance with the Air Force Records Disposition Schedule, which is located in the Air Force Records Information Management System.
