DAFMAN91-221 covers safety investigation personnel, test and evaluation mishaps, report preparation, classified safety reporting, and follow-up actions. It requires scheduled preliminary, status, and final messages, including reporting through AFSAS and SIPRNet workflows.
Applies to: This publication applies to all civilian employees and uniformed members of Regular Air Force (USAF), the Air National Guard, the Air Force Reserve, the United States Space Force (USSF), and those with a contractual obligation to abide by the terms of DAF publications.
Required when investigation involves a fatality, otherwise the requirement is optional. (paragraph 2)
Qualifying test events may be investigated using a SIB, Launch Analysis Group (LAG) or Air Launched Munitions Analysis Group (ALMAG). (paragraph 4)
If the investigation will not be complete within the time frame, transmit 30-day status message on day 30 and every 30 days until investigation is complete. Include estimated completion date. (paragraph 6)
Do not delay final messages awaiting testing results. If the testing results significantly change the final message’s outcome, reconvene the SIB (if necessary) and send a status message describing changes. (paragraph 7)
Reports shall include any additional report (e.g., Deficiency Reports (DR), Unsatisfactory Reports (UR), AFTO Form 22) reference number, if applicable. (paragraph 2)
