It integrates ASAP components with the Air Force Safety Automated System and defines required information for ASAP reports. It establishes display, de-identification, mishap processing, duplicate-report, and initial-inquiry procedures.
Applies to: This guidance is applicable to the United States Air Force and the United States Space Force.
Required fields include the event narrative, date and time, aircraft type (if applicable), wing/delta, and the relevant airfield.
The SAFEREP Display shows the report narrative, submitter-recommended actions, and the MAJCOM or wing/delta’s resolution.
Access requires a Common Access Card but does not require an AFSAS account.
Submissions received via ASAP that indicate damage, injury, or occupational illness are mishap reports.
As ASAP processing includes de-identification, names and other information that may enable identification of personnel involved will not be visible on the SAFEREP Display.
