It provides common guidance for investigating and reporting Department of the Air Force safety events and revises reporting requirements in AFSAS. The publication requires compliance with mandatory provisions, identifies waiver authorities, and addresses records and Privacy Act information.
Applies to: It applies to all Regular Air Force (AF) and personnel assigned to AFGSC.
Failure to observe the prohibitions and mandatory provisions in paragraph 4.4. of DAFI 91-204 by Regular AF members is a violation of Article 92, Uniform Code of Military Justice (UCMJ).
The authorities to waive wing/unit level requirements in this publication are identified with a Tier (“T-0, or T-3”) number following the compliance statement.
Submit requests for waivers through the chain of command to the appropriate Tier waiver authority, or alternately, to the requestor’s commander for non-tiered compliance items.
Changes include the addition of reporting requirements for Launch Control Centers (LCC) for events that cause evacuations or other precautions and updates Budget and Operations guidance.
