DAFI48-122 addresses deployment health activity requirements, roles for medical and public health personnel, and procedural guidance for deployment-related health assessments. It requires protected information to be collected and maintained, affected forms to include Privacy Act statements, and resulting records to be managed and disposed of according to Air Force requirements.
Applies to: It applies to Regular Air Force (RegAF), Air Force Reserve (AFR) and Air National Guard (ANG), and Department of the Air Force (DAF) civilians at all levels that deploy (Note: ANG and AFR will be collectively referred to as Air Reserve Component (ARC) except where noted otherwise).
Forms affected by the Privacy Act must have an appropriate Privacy Act statement.
Ensure all records created as a result of processes prescribed in this publication are maintained in accordance with Air Force Instruction 33-322, Records Management and Information Governance Program, and disposed of in accordance with the Air Force Records Disposition Schedule located in the Air Force Records Information Management System.
Refer recommended changes and questions about this publication to the OPR listed above using the AF Form 847, Recommendation for Change of Publication; route AF Forms 847 from the field through the appropriate functional chain of command.
Submit requests for waivers through the chain of command to the appropriate Tier waiver approval authority, or alternately, to the publication OPR for non-tiered compliance items.
