Why must the attached page inserts be removed and replaced?

Due to administrative oversight, the proficiency codes were incorrectly identified, and training line numbers were misnumbered.

Which training references were omitted from the CFETP?

Lastly, two training references were left off of the Magnetic Resonance Imaging (C-shred) prior to publication of the 4R0X1/A/B/C CFETP.
