It establishes annual recognition for exemplary contributions and dedication to the SE&TM community through awards for individuals and teams. Nomination packages require specified forms, citations, endorsements, narrative-style Performance Statements, and unclassified information.
Applies to: This publication applies to all AFMC Regular Air Force.
All nominations require a cover sheet, citation, and an AF Form 1206, Nomination for Award, which addresses each criteria element. (paragraph 1)
Teams should not exceed 35 members, and members can be listed on a separate page. (paragraph 1)
Contractor personnel may not be nominated for an individual award or included as a nominee in a team award. (paragraph 2)
All nominations must be written in narrative-style Performance Statements, using Times New Roman, 12 point font. (paragraph 3)
Each Performance Statement must be a stand-alone sentence and include at least two of the following: action, impact, and/or outcome. (paragraph 3)
