Applies to: It applies to commanders, functional managers, supervisors and all USAF owned assets.
Report all of the following events located in Tables 3.1 through Table 3.5 of AFMAN91-223, whether intent for flight is established or not, within 3 hours using 18 WG Form 82, 18th Wing Aviation Incident Initial Report.
Send the completed 18 WG Form 82 to 18 WG/SEF, 18th Operations Group Commander (18 OG/CC), 18th Maintenance Group Commander (18 MXG/CC), Squadron Commander (SQ/CC), and Squadron Safety Office.
18th Wing Safety (18 WG/SEF) is the office of primary responsibility for reviewing and reporting safety mishaps and incidents at Kadena AB.
Ensure that all records created as a result of processes prescribed in this publication are maintained in accordance with (IAW) Air Force Manual (AFMAN) 33-363, Management of Records, and disposed of IAW Air Force Records Information Management System (AFRIMS) Records Disposition Schedule (RDS).
