The publication requires each squadron to appoint a report of survey monitor and alternate and defines the monitor’s duties. It also addresses equipment accountability during separation and depreciation methods for Air Force ADPE.
Each squadron must appoint, in writing, a report of survey monitor and alternate.
The monitor will ensure all necessary blocks are completed and basic questions (who, what, when, where, why and how) are answered.
The investigation needs to include all available statements made by involved individuals or witnesses.
Clearance through Base Supply for individual equipment items should be accomplished no less than 30 days prior to final separation date to allow for completion of report of survey for any lost or damaged items.
Depreciation on Air Force ADPE equipment is applied with a five year average life.
