All Forms 8 and FEFs will be created, maintained, and routed via PEX.
Evaluators will make comments on the form when the overall grade is EQ or for any area/sub area graded commendable, Q-, or U.
Trends to be tracked are: 1) 20% or more of an area/sub area is graded a Q- or lower with a sample size of 10 or more; 2) a question on a requisite or periodic exam is missed 20% or more of the time, with a sample size of 10 or more.
Formal periodic exams will consist of 25 questions and will be administered semiannually.
