8FWVA91-2E covers accident and injury reporting, notification checklists, serious injuries, and routing through supervisors, commanders, and safety representatives. It requires immediate notification for specified serious injuries and submission of AF Form 978 within five days.
In the event of an accident or injury, seek medical attention immediately and/or notify your immediate supervisor, or commander in absence of supervisor. (paragraph 91-204)
The supervisor or commander then notifies the Command Post/Maintenance Operations Center (as applicable) and the USR. (paragraph 91-204)
For major accidents that involve loss of life, limb, eyesight, medical transport, or other serious injuries, notify Command post who will then run applicable notification checklists and notify the applicable commanders and wing safety. (paragraph 91-204)
Supervisors will generate an AF Form 978 within five days, forward to USR, and the USR will route the Form 978 through the unit commander then to wing safety. (paragraph 91-204)
