How far in advance must a Base Use Request Sheet be submitted?

Provide a Base Use Request Sheet with all information required, no later than (NLT) 30 days prior to the event start date via e-mail. (paragraph 3)

What happens if required event documentation is not provided?

Failure to provide required documentation and/or information will result in a revoked approval. (paragraph 3)

What information must be included for non-military event participants?

Entry Access List (EAL) and Standard Form (SF) 2 identifying full name, social security number, driver’s license number and date of birth for all non-military participants. (paragraph 3)

When is an Entry Access List not needed?

An EAL is not needed for personnel that possess valid government credentials per DoDM 5200.08v3 AFMAN 31-101v3, Physical Security Program: Access to DoD Installations, 2 January 2019. (paragraph 3)

How quickly must functional points of contact respond to pre-approval notifications?

Functional POCs must respond within 3 days following notification or the event will be considered approved. (paragraph 3)
