It provides guidance for preparing AE malfunction reports and adds guidance for ammunition, missile, and rocket malfunction report forms. It addresses reporting duds and misfires, preliminary reports, malfunction sites, investigations, and suspended ammunition.
To provide guidance for conducting investigations and preparing ammunition and explosives (AE) malfunction reports (DA Form 4379 (series)). (paragraph 1-1)
Immediately cease firing suspected standard and nonstandard ammunition and notify range control or equivalent. (paragraph 2-1)
All duds and misfires are reportable and will be brought to the attention of the local ammunition officer and QASAS. (paragraph 2-1)
Dud and misfire will be reported using the same procedures as for other types of malfunctions. (paragraph 2-1)
Training may be continued, if permitted by the local QASAS or range officer, local range standard operations, host nation policies, Resource Conservation and Recovery Act and/or Environmental Protection Agency State environmental permits, military munitions rule, and applicable technical manuals. (paragraph 2-1)
