When does AR 635-8 require a DD Form 214 to be prepared?

The DD Form 214 is a summary of the Soldiers most recent period of continuous active duty. It provides a brief, clearcut record of all current active, prior active, and prior inactive duty service at the time of REFRAD, retirement, or discharge. (paragraph 1)

When can a transition center reissue a DD Form 214?

Once a DD Form 214 has been issued, transition centers do not reissue except for the following reasons: (1) When directed by appellate authority, executive order, or by the Secretary of the Army. (2) When it is determined that the original DD Form 214 cannot be properly corrected by issuance of a DD Form 215. (paragraph 2)

What happens if a Soldier loses a DD Form 214?

Do not issue DD Form 214 to replace copies or DD Forms 214 lost by the Soldier. If no DD Form 214 is available, issue DA Form 1569 (Transcript of Military Record). (paragraph 2-15)

When must a new DD Form 214 be issued instead of a DD Form 215?

Do not issue a DD Form 215 when more than one page will be required to complete the correction. Administratively issue a new DD Form 214. (paragraph 4)

How must DD Form 215 data be transcribed?

Data for items 1, 2, 3, and 4 of the DD Form 215 must be transcribed exactly as it appears on the DD Form 214 being corrected, even if the data contains typographical errors. (paragraph 2-14)
